well if it loosens up again get the bike up on a stand so the front wheel is off the ground,back off both nuts for the steering stem as much as you can,, let the fork drop down a bit out of the frame head and see if the stem is pulling out,,should be easy enough to tell if it's moving,,,and it's not a crazy fix either,,,
